Indication
Used as a mucolytic agent in the treatment of chronic obstructive pulmonary disease (COPD) and as an antidote for acetaminophen overdose.
Dosage (summary)
The usual adult dose is 600 mg to 1200 mg per day, divided into two or three doses. For acetaminophen overdose, the loading dose is 140 mg/kg followed by 70 mg/kg every 4 hours for 17 doses.
Onset of Action / Duration
Onset of action is typically within 1 hour for mucolytic effects; for acetaminophen overdose, therapeutic effects can be observed within 8 hours.
Special Populations
- Elderly patients
- Patients with hepatic impairment
- Patients with renal impairment
Pregnancy & Breastfeeding
N-Acetylcysteine is classified as Category B. It is generally considered safe during pregnancy and lactation, but should be used under medical supervision.
Key Drug Interactions
- Nitroglycerin - may enhance the vasodilatory effects, leading to increased risk of hypotension.
- Activated charcoal - may reduce the absorption and effectiveness of N-Acetylcysteine.
Contraindications
- Hypersensitivity to N-Acetylcysteine or any of its components
- Severe asthma or bronchospasm

- Diarrhea
- Abdominal pain
- Rash
- Anaphylactic reactions (rare)
Counselling Points
- Advise patients to take with food to minimize gastrointestinal upset.
- Instruct patients to report any signs of allergic reactions, such as rash or difficulty breathing.
- Inform patients about the importance of adhering to the prescribed dosage regimen.
Serious warnings
- Use with caution in patients with asthma or a history of bronchospasm.
- Monitor liver function in patients with hepatic impairment.
- May cause drowsiness; caution patients about driving or operating machinery.
