Indication
Prophylaxis and chronic treatment of asthma in adults and children aged 2 years and older; relief of symptoms of seasonal allergic rhinitis.
Dosage (summary)
10 mg once daily in the evening for adults and children aged 15 years and older; 5 mg once daily for children aged 6 to 14 years; 4 mg once daily for children aged 2 to 5 years.
Onset of Action / Duration
Onset of action within 24 hours; maximum effect may take several days.
Special Populations
- Hepatic impairment
- Renal impairment
- Elderly patients
Pregnancy & Breastfeeding
Category B; use only if clearly needed. Montelukast is excreted in breast milk, caution is advised.
Key Drug Interactions
- Phenobarbital may decrease montelukast levels.
- Rifampicin may decrease montelukast levels.
- Warfarin may have altered effects when used with montelukast.
Contraindications
- Hypersensitivity to montelukast or any of the excipients.
Common side effects
- Headache
- Gastrointestinal disturbances (nausea, diarrhea)
- Fatigue
- Dizziness
- Mood changes (including agitation, aggression, depression)
Counselling Points
- Take montelukast in the evening for optimal effect.
- Do not use montelukast as a rescue medication for acute asthma attacks.
- Report any mood changes or unusual behavior to a healthcare provider.
Serious warnings
- Caution in patients with a history of psychiatric disorders.
- Monitor for signs of Churg-Strauss syndrome in patients with asthma.
- Not a substitute for corticosteroids in the management of asthma.
