Dosage (summary)
Initial: 10-15 mg daily for 3 days; Maintenance: 2.5-10 mg daily.
Onset of Action / Duration
Onset: 24-36 hours, Duration: 2-5 days

- Elderly
- Renal impairment
- Hepatic impairment
Pregnancy & Breastfeeding
Contraindicated in pregnancy and breastfeeding.

Contraindications
- Hypersensitivity to warfarin
- Haemorrhagic states
- Pregnancy
- Severe liver impairment
Common side effects
- Haemorrhage
- Nausea
- Diarrhoea
- Rash
Counselling Points
- Take at the same time daily
- Avoid sudden dietary changes
- Report any signs of bleeding
Serious warnings
- Risk of serious bleeding
- Monitor INR regularly
