Indication

Prevention and management of thromboembolic disorders.

Dosage (summary)

Initial: 10-15 mg daily for 3 days; Maintenance: 2.5-10 mg daily.

Onset of Action / Duration

Onset: 24-36 hours, Duration: 2-5 days

Special Populations

  • Elderly
  • Renal impairment
  • Hepatic impairment

Pregnancy & Breastfeeding

Contraindicated in pregnancy and breastfeeding.

  • NSAIDs
  • Antibiotics
  • Herbal products (St John's Wort)

Contraindications

  • Hypersensitivity to warfarin
  • Haemorrhagic states
  • Pregnancy
  • Severe liver impairment

Common side effects

  • Haemorrhage
  • Nausea
  • Diarrhoea
  • Rash

Counselling Points

  • Take at the same time daily
  • Avoid sudden dietary changes
  • Report any signs of bleeding

Serious warnings

  • Risk of serious bleeding
  • Monitor INR regularly
