Initial IV bolus of 7-15 u03bcg/kg for short procedures; adjust for age and condition.

Pregnancy & Breastfeeding
Not established safe in pregnancy; avoid during childbirth; may enter breast milk.

Contraindications
- Intolerance to opioids
- Respiratory depression risk
- Myasthenia gravis

- Ensure monitoring equipment available
- Have naloxone ready
- Avoid driving for 6-24 hours post-dose
