Indication
Prophylaxis and chronic treatment of asthma in adults and children, and for the relief of symptoms of seasonal allergic rhinitis.
Dosage (summary)
Adults and children 15 years and older: 10 mg once daily in the evening. Children 6 to 14 years: 5 mg once daily in the evening. Children 2 to 5 years: 4 mg once daily in the evening. Chewable tablets should be chewed before swallowing.
Onset of Action / Duration
Onset of action is within 1 hour; duration of effect is 24 hours.

- Hepatic impairment
- Renal impairment
- Elderly patients
Pregnancy & Breastfeeding
Montelukast should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; caution should be exercised when administered to nursing mothers.
Key Drug Interactions
- Phenobarbital may decrease montelukast levels.
- Rifampicin may decrease montelukast levels.
- Concomitant use with other medications that induce CYP450 enzymes may reduce effectiveness.
Contraindications
- Hypersensitivity to montelukast or any of the excipients.
Common side effects
- Headache
- Gastrointestinal disturbances (nausea, diarrhea)
- Fatigue
- Dizziness
- Mood changes (including agitation, aggression, anxiety, depression)
Counselling Points
- Take the medication in the evening for optimal effect.
- Do not use as a rescue medication for acute asthma attacks.
- Inform healthcare provider if experiencing mood changes or unusual behavior.
- Continue taking even if symptoms improve, unless directed otherwise by a healthcare provider.
Serious warnings
- May cause neuropsychiatric events; monitor for changes in behavior or mood.
- Use with caution in patients with a history of depression or other psychiatric disorders.
- Not a substitute for corticosteroids or other asthma medications.
