Indication

Vitamin B12 deficiencies including megaloblastic anaemia.

Dosage (summary)

250 u03bcg to 1,000 u03bcg IM on alternate days for 1-2 weeks, then 250 u03bcg weekly; maintenance 1,000 u03bcg monthly.

Special Populations

  • Elderly
  • Patients with renal impairment

Pregnancy & Breastfeeding

Not for megaloblastic anaemia in pregnancy; secreted in breast milk.

Key Drug Interactions

  • Antibiotics
  • Nitrous oxide
  • Oral contraceptives
  • Folic acid
  • Vitamin C

Contraindications

  • Hypersensitivity to vitamin B12
  • Toxic amblyopias
  • Megaloblastic anaemia of pregnancy
  • Benzyl alcohol in neonates

Common side effects

  • Dizziness
  • Nausea
  • Injection site reactions
  • Hypersensitivity reactions

Counselling Points

  • Avoid driving until effects are known
  • Report any allergic reactions
  • Follow-up for blood count monitoring

Serious warnings

  • Monitor potassium levels during initial therapy
  • Evaluate long-term therapy every 6-12 months
