Indication
Vitamin B12 deficiencies including megaloblastic anaemia.
Dosage (summary)
250 u03bcg to 1,000 u03bcg IM on alternate days for 1-2 weeks, then 250 u03bcg weekly; maintenance 1,000 u03bcg monthly.
Special Populations
- Elderly
- Patients with renal impairment
Pregnancy & Breastfeeding
Not for megaloblastic anaemia in pregnancy; secreted in breast milk.

- Antibiotics
- Nitrous oxide
- Oral contraceptives
- Folic acid
- Vitamin C
Contraindications
- Hypersensitivity to vitamin B12
- Toxic amblyopias
- Megaloblastic anaemia of pregnancy
- Benzyl alcohol in neonates
Common side effects
- Dizziness
- Nausea
- Injection site reactions
- Hypersensitivity reactions
Counselling Points
- Avoid driving until effects are known
- Report any allergic reactions
- Follow-up for blood count monitoring
Serious warnings
- Monitor potassium levels during initial therapy
- Evaluate long-term therapy every 6-12 months
