Indication
Rapid corticosteroid effect in severe shock and acute hypersensitivity reactions.
Dosage (summary)
Initial IV dose: 100 mg to 500 mg over 30 seconds to 10 minutes; repeat every 4-6 hours as needed.

Onset: 1 hour, Duration: variable
Special Populations
- Elderly
- Children
- Renal impairment
- Hepatic impairment
Pregnancy & Breastfeeding
Crosses placenta; may cause fetal abnormalities; caution in breastfeeding due to potential adrenal suppression.
Key Drug Interactions
- Ciclosporin (risk of seizures)
- CYP3A inhibitors (increased side effects)
- Diuretics (risk of hypokalaemia)
- Antidiabetic agents (dosage adjustment needed)
Contraindications
- Hypersensitivity to hydrocortisone
- Systemic fungal infections
- Active tuberculosis
- Live vaccines in immunosuppressed patients
Common side effects
- Increased blood pressure
- Hyperglycemia
- Cushing's syndrome
- Osteoporosis
- Psychiatric disturbances
Counselling Points
- Monitor for signs of infection
- Avoid live vaccines during treatment
- Report any psychological symptoms immediately
- Gradual dose reduction to prevent withdrawal syndrome

- Risk of adrenal insufficiency on withdrawal
- Potential for severe psychiatric reactions
