Indication

Dosage (summary)
2.5 mg twice daily for VTE; 5 mg twice daily for NVAF.
Special Populations
- Renal impairment
- Hepatic impairment

Pregnancy & Breastfeeding
Avoid in pregnancy; unknown if excreted in breast milk.

- Strong CYP3A4 and P-gp inhibitors
- Anticoagulants
- Antiplatelet agents
Contraindications
- Hypersensitivity to apixaban
- Active bleeding
- Severe renal disease
- Severe hepatic impairment

- Haemorrhage
- Contusion
- Epistaxis
- Haematoma

- Take with or without food
- Monitor for signs of bleeding
- Do not crush tablets unless necessary
