Indication
Control of sustained ventricular dysrhythmias and paroxysmal atrial fibrillation.
Dosage (summary)
Starting dose: 50 mg twice daily for supra-ventricular; 100 mg twice daily for ventricular. Max: 300 mg or 400 mg daily.
Onset of Action / Duration
Onset: 2-3 hours, Duration: 12-14 hours
Special Populations
- Elderly
- Renal impairment
- Hepatic impairment
Pregnancy & Breastfeeding
Not established; avoid in pregnancy, especially first trimester.
Key Drug Interactions
- Increased digoxin levels
- Additive effects with beta-blockers
- Avoid with other class 1 anti-dysrhythmics
Contraindications
- Cardiac failure
- Recent myocardial infarction
- Atrial fibrillation
- Sinus node dysfunction
Common side effects
- Dizziness
- Visual disturbances
- Pro-dysrhythmic effects
Counselling Points
- Monitor for dizziness
- Report visual changes
- Avoid in pregnancy
Serious warnings
- Electrolyte disturbances
- Caution in hepatic impairment
- Increased mortality in post-MI patients
