Adults: 400 mg on day 1, then 200 mg daily; adjust for renal impairment.

Avoid in pregnancy unless benefits outweigh risks; not recommended during breastfeeding.

- Hypersensitivity to fluconazole
- Co-administration with terfenadine at high doses
