Dosage (summary)

Adults: 4 g/0.5 g every 8 hours; adjust for renal impairment.

Special Populations

  • Renal impairment
  • Elderly

Pregnancy & Breastfeeding

Use only if benefits outweigh risks; crosses placenta and low levels in breast milk.

  • Probenecid
  • Aminoglycosides
  • Methotrexate

Contraindications

  • History of allergic reactions to penicillins or cephalosporins
