Indication
Antidote to folic acid antagonists like methotrexate.
Dosage (summary)
Administer 75 mg IV within 12 hours of methotrexate, then 12 mg IM every 6 hours for 4 doses.
Special Populations
- Renal impairment
- Gastrointestinal obstruction
Pregnancy & Breastfeeding
Safety in pregnancy and lactation not established.
Key Drug Interactions
- Enhances toxicity of fluorouracil
- May increase seizures with barbiturates
Contraindications
- Hypersensitivity to folinic acid
- Vitamin B12 deficiency
- Stomach and bowel complaints

- Take as soon as possible after methotrexate overdose
- Monitor plasma methotrexate levels

- Not for pernicious anemia treatment
