Indication
Anticoagulant for arterial and venous thrombosis treatment.
Dosage (summary)
IV: Initial 12,500 units, then 10,000 units every 4 hours; SC: 5,000 units every 8-12 hours for prophylaxis.
Special Populations
- Elderly
- Renal impairment
- Hepatic impairment
Pregnancy & Breastfeeding
Safer than warfarin in pregnancy; does not cross placenta or enter breast milk.

- Hypersensitivity to heparin
- Active hemorrhage
- Severe hypertension
