Indication

Second line treatment of acute and chronic bronchial obstruction.

Dosage (summary)

Loading: 6 mg/kg lean body mass IV; Maintenance: 0.4-0.5 mg/kg/hour for non-smokers.

Special Populations

  • Elderly
  • Hepatic impairment
  • Renal impairment
  • Congestive cardiac failure

Pregnancy & Breastfeeding

Safety not established; monitor neonates for apnoea and irritability.

Key Drug Interactions

  • Fluvoxamine may increase theophylline levels.
  • Smoking increases theophylline clearance.
  • Cimetidine may raise theophylline levels.

Contraindications

  • Hypersensitivity to aminophylline or xanthines.
  • Porphyria.
  • Active gastritis or peptic ulcer disease.

Common side effects

  • Nausea
  • Vomiting
  • Headache
  • Tachycardia
  • Dizziness

Counselling Points

  • Avoid rapid IV injection.
  • Monitor for signs of toxicity.
  • Do not alter dose without consulting a doctor.
